| The completed Nomination Forms, the reflection of teaching practices and the DVDs/CDs should be submitted in triplicate (including the original), in person or by post, to the Chief Executive’s Award for Teaching Excellence Secretariat at Room 1107, 11/F., Wu Chung House, 213 Queen’s Road East, Wan Chai, Hong Kong on or before 5 October 2018. For nominations submitted by post, date of the stamp chop will be used as the submission date. |
