| The completed Nomination Forms, the reflection of teaching practices and the DVDs/CDs should be submitted in triplicate (including the original), in person or by post, to the Chief Executive’s Award for Teaching Excellence Secretariat, 11/F., Wu Chung House, 213 Queen’s Road East, Wan Chai, Hong Kong on or before 7 October 2016. For postal nomination, date of the stamp chop will be used as the submission date. |
