| The completed Nomination Forms and all the enclosures should be submitted in triplicate (including the original), in person or by post, to the Chief Executive’s Award for Teaching Excellence Secretariat, 11/F., Wu Chung House, 213 Queen’s Road East, Wan Chai, Hong Kong on or before 3 October 2014. For postal nomination, date of the stamp chop will be used as the submission date. |
